Registration: Please complete and send/fax the registration form below. Registration AND payment of $1,295 USD must be received by April 1, 2006. Payment may be made by check or by wire transfer. An invoice with wire transfer instructions will be sent shortly after registration. An early registration fee of $1,195 will be honored if registration AND payment are received by March 1, 2006. Hotel: Please note that you are responsible for your hotel reservations and payment at the Novotel Basel. PTC has reserved a block of 20 guest rooms at a group rate of 220.20 CHF per night, so please make your reservations as soon as possible, and no later than April 1, 2006. Meals: Lunch and coffee breaks on both days of the course will be provided (included in the course fees). If you have dietary restrictions (medical, religious or otherwise), please let us know by April 1 and we will be able to order a vegetarian lunch for you.
